HIPAA Notice of Information Practices and Privacy Statement.

THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.

As required by the Privacy Regulations Promulgated Pursuant to the Health Insurance Portability and Accountability Act of 1996 (HIPAA) THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY. This Notice of Privacy Practices describes how Talent Testing Service, Inc. (TTS) may use and disclose your "protected health information" (PHI) to carry out diagnosis and treatment, provide clearance status, and for other purposes that are permitted or required by law. It also describes your rights to access and control your PHI. PHI is information about you, including demographic information, that may identify you and that relates to your past, present, or future health and related health care services.

Uses and Disclosures of PHI:

Your PHI may be used and disclosed by TTS staff for the purpose of providing health care services to you, such as lab services, diagnosis, and treatment. Additionally, your PHI will be shared when required by law, such as to the mandatory reporting of the occurrence of reportable infectious diseases. Associated companies, such as draw facilities, are given only enough information to provide necessary services to you. All patients are required to complete a Medical History Questionnaire (MHQ). This is a focused assessment of each patient's general and sexual health. Information obtained in the MHQ is reviewed only by TTS staff (the MHQ is not shared with draw facilities, agents, producers nor the Free Speech Coalition (FSC)).

Treatment:

If deemed appropriate by our Medical Director, we will use and disclose your PHI to diagnose and treat you. In instances where TTS is unable to provide complete treatment, your PHI and lab results may be sent to a pharmacy or health care facility, of your choosing, that will be able to carry out the request for treatment. In the case of a positive, reactive, or unexpected result, your laboratory results will be made available, to you, via an encrypted HIPAA compliant system.

Healthcare Operations:

Your PHI may be used or disclosed in order to support the laboratory and/or healthcare activities of our organization. These activities include, but are not limited to, quality assessment activities, employee review activities and accreditation activities. We may also call you by name while you are at our facility or use your name and other demographics to contact you with test results. We will not disclose test results or other information without your consent (except under certain circumstances, which are listed below).

Disclosure of PHI:

There are certain situations in which your PHI may be disclosed without your prior authorization in the following situations: uses and disclosures Required By Law, Public Health Authorities (State and local health departments, the Food and Drug Administration (FDA), the Centers for Disease Control and Prevention, Occupational Safety and Health Administration (OSHA), Health Oversight Activities, Victims of Abuse, Neglect or Domestic Violence, Judicial and Administrative Proceedings, Law Enforcement Purposes, Research Purposes, Specialized Government Functions, and Workers' Compensation. Under the law, we must make disclosures to you and when required by the Secretary of the Department of Health and Human Services to investigate or determine our compliance with the requirements of Section 164.500.

Your Rights:

You have the right to inspect and/or request a copy of your PHI. You have the right to request restricted access to your PHI. You have the right to obtain a paper or electronic copy of this notice from us. You have the right to have your PHI amended if information is incorrect or has changed. TTS reserves the right to change the terms of this notice and will inform you by email of any major changes. You then have the right to object or withdraw as provided in this notice.
